ProblemThe Language Gap Nobody Was Solving
For a Neuropsychiatry Hospital in Gujarat, India the challenge was not clinical. It was communicational.
The hospital serves approximately 50 patients a day — largely rural, working-class individuals managing conditions such as depression, epilepsy, and anxiety. These are patients for whom continuity of care is not optional; a missed follow-up appointment carries real clinical consequences.
Yet the clinic's appointment reminder workflow had a fundamental flaw: every reminder was sent in English, to a patient base that was almost entirely Gujarati-speaking. The friction points were compounded by the nature of the existing process:
- Manual follow-up calls consumed significant front-desk staff time each day, pulling attention away from in-clinic patient management.
- No-show rates remained persistently high, with patients citing confusion around appointment details — dates, times, and doctor names — as common reasons for missing visits.
- Generic SMS templates from off-the-shelf tools were transactional, English-only, and wholly inappropriate for a psychiatric practice where the tone of patient communication directly influences trust and engagement.
For a neuropsychiatry practice, a missed appointment is not simply lost revenue. It is a gap in a patient's care plan — one that can have serious downstream consequences for vulnerable individuals.
SolveAutomate Communication in the Patient's Own Language
The mandate for Madspek was precise: eliminate manual reminder workflows, communicate with patients in Gujarati, and deploy a solution that required no disruption to the clinic's existing systems or staff workflows.
The goal was not to build another dashboard or reporting tool. It was to put an intelligent, empathetic communication agent directly into the clinic's daily operations — one that could speak to patients the way a trained staff member would, at scale, without adding to the administrative burden.
The AI ArchitectureDesigning an AI-Native Workflow on Existing Tools
Madspek began by working closely with the clinic's operational workflow to understand how patient data was captured, stored, and accessed. Rather than mandating a complex EMR integration, Madspek identified a Cloud-Native Data Layer with Patient Data Privacy at Its Core — a tool the clinic already used, requiring zero onboarding or retraining.
This informed the design of MedVaani — an autonomous, stateful patient communication agent built on a lightweight agentic pipeline.
| Component | Operational Role | Technology |
|---|---|---|
| Data Ingestion Agent | Reads pending appointments from the data layer, filtering only uncontacted patients. | gspread + Google Sheets API |
| Vernacular Generation Engine | Generates empathetic, contextually appropriate Gujarati reminders using a large language model. | Gemini 2.5 Flash via REST API |
| Dispatch Layer | Delivers Unicode SMS to patient mobile numbers, including basic feature phones with no internet dependency. | Fast2SMS Gateway |
| Audit & State Agent | Writes delivery status back to the source, preventing duplicate sends and maintaining a live communication log. | gspread write-back |
The Vernacular Generation Engine
The core intelligence of MedVaani lies in its prompt architecture. Rather than relying on rigid translated templates, the agent sends structured appointment data to Gemini 2.5 Flash with a carefully designed system prompt that instructs the model to generate a warm, natural Gujarati reminder — one that reads as written by a human, not produced by a machine.
An example of a generated patient reminder:
નમસ્તે પ્રાચી બેન, ડૉ. ધર્મિન શાહ સાથે આપની એપોઇન્ટમેન્ટ ૨૧/૫/૨૦૨૬ ના બપોરે ૩ વાગ્યે છે. મળીશું.
Translation: "Hello Prachi, your appointment with Dr. Dharmin Shah is on 21/5/2026 at 3:00 PM. See you then."
Each message is generated fresh per patient — personalized by name, date, and time — rather than populated into a fixed template. The result is communication that feels personal rather than automated.
Stateful Automation
A critical design decision was making the agent stateful. After each successful dispatch, MedVaani writes a "Sent" status back to the corresponding row in the data layer. This prevents duplicate messages from being sent on subsequent runs and gives the clinic a real-time audit trail of all patient communications — directly inside the tool they already use.
The entire pipeline — from reading the appointment data to dispatching reminders for a full day's appointments — completes in under two minutes.
ResultsFaster Communication, Zero Disruption
The deployment at Manas Neuropsychiatry Hospital transformed a fragmented, manual process into a fully automated vernacular communication pipeline.
Eliminated Manual Reminder Workflows
Front-desk staff were entirely removed from the reminder dispatch process. The agent handles identification, generation, and delivery autonomously — returning staff time to in-clinic patient management during peak hours.
Vernacular Communication at Scale
Every patient now receives an appointment reminder in Gujarati — the language they actually read and respond to. At 50 patients per day, this represents a meaningful shift in how the clinic reaches its patient base, particularly for elderly and rural individuals who were previously unreachable through English-language communication.
Zero Disruption to Existing Systems
No EMR replacement. No new software for staff to learn. No IT infrastructure changes. MedVaani operates as an invisible layer on top of the clinic's existing workflow — adding intelligence without adding complexity.
A Foundation for What Comes Next
The pilot establishes the infrastructure for a significantly expanded communication stack — including voice call delivery in Gujarati for patients unreachable by SMS, two-way scheduling via vernacular chatbot, and direct EMR integration as the deployment matures.
